Mobile Dental Services Period of Performance is projected to be: 1 Feb 2020 - 30 Jan 2025 Base plus four one year option periods. Dental Readiness, FFP In support of Individual Medical Readiness (IMR) Including Exams, Treatment and DENCLASS Entry. Contractor shall provide all management, labor, material, equipment, certifications and supplies necessary to provide all listed Dental services, utilizing a HIPAA, Privacy Act, OSHA and ADA Compliant mobile health facility or portable equipment in accordance with the Performance Work Statement. (PWS), which is attached and made part of this solicitation. Complete Solicitation will be posted on or about 1 Dec 2019 The solicitation is issued as a Request for Quote (RFQ) from the United States Property and Fiscal Office for New York, Purchasing and Contracting Division, Latham, New York. Principle NAICS is 621210 and size standard is $7,500,000.00. The New York Army This acquisition is utilizing procedures in FAR part 13, Simplified Acquisition Procedures. Medical Readiness Event (MRE) predominately occurs at Camp Smith, NY were the vendor is required to setup of contractor’s dental equipment to be completed either the day prior to event or day of the event provided they can be fully operational and able to take patients at least 1/2 hour prior to the start time of each event. Staffing and equipment levels is estimated at no less than 9 dentists and supporting staff per full day and 6 dentists and supporting staff for a half ½ day. Contractor Must Provide proof of its authorization through the New York Secretary of State and New York State Education Department as an entity registered to provide dental services within the State of New York. The resulting contract will require the Contractor to be compliant with the Contract Manpower reporting (CMR) system. The Contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the NYARNG via a secure data collection site. See attached Statement of Work for more information. In order to be eligible for award, the successful offeror's System for Award Management (SAM) registration must be active and includes representations and certifications. This solicitation will be issued electronically on Contracting Opportunities, website https://beta.sam.gov, therefore hardcopies will not be provided. Upon release of the solicitation, it will be available on the https://beta.sam.gov. CAUTION: This solicitation will be issued electronically as will any amendments thereto. Because of this, the Government is under no obligation and is in fact unable to maintain a bidder's mailing list. It is therefore incumbent upon any interested parties to periodically access the above Internet address in order to obtain any amendments that may be issued. Failure to obtain any said amendments and to respond to them prior to the date and time set for receipt of offers may render your offer nonresponsive and result in the rejection of same.
