THIS SOURCES SOUGHT NOTICE DOES NOT CONSTITUTE A REQUEST FOR PROPOSAL, REQUEST FOR QUOTE, OR INVITATION FOR BID. This notice is to make the industry aware of an upcoming requirement. Please be advised that this requirement be delayed, revised, or cancelled at any time prior to an award being made. The North American Industry Classification System Code (NAICS) is 621111, Offices of Physicians (Except Mental Health Specialists). The Government intends on issuing one service contract. Character and Extent of Work: This project includes various forms of services such as: Fit for Duty physical exam, covering employees who are assigned to/as divers, crane operators, seagoing vessels, working at hazardous waste sites, overseas deployments, new hires, and post injury return to work exams. Additional tests may be based on U. S. Army Corps of Engineers (USACE) requirements and medical doctor's recommendations. All medical exams shall be performed by a qualified occupational health physician, a licensed and board eligible/board certified doctor of medicine, or osteopathy, and who, by virtue of education, knowledgeable of regulations and resources relating to occupational medicine such as Occupational Safety and Healthy Administration (OSHA), National Institute for Occupational Safety and Healthy (NIOSH), Department of Defense (DOD), Department of the Army (DA) and American College of Occupational and Environmental Medicine (ACOEM) guidelines. Official Solicitation will likely be Sought shall include: -Offeror's name, address, point of contact, phone number, and e-mail address. -Offerors' business designation (eg. Large, Small, 8(a), etc). -Commercial and Government Entity (CAGE) code. -Offeror's interest in bidding on the solicitation once it's issued. -Offeror's capability to meet the requirements including specific equipment and experience. -Offeror's capability to perform a contract of this complexity, comparable work performed within the past 5 years, including a brief description of the project, customer name, timelines of performance, customer satisfaction, and dollar value of project.
