The Puerto Rico Army National Guard Medical Command has a requirement to acquire the service of a Company that provide all necessary contractors, needed by the government, to Complete Full Periodic Health Assessment, Soldiers Readiness Program and/or Physical Exam Requirements. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only notice. Attached is a written solicitation with all the specifications and requirements of the equipment. This requirement is a request for proposal (RFP). The solicitation documents and incorporated provisions and clauses are those in effect in the Federal Acquisition Circular 2021-5. The award will be a Firm Fixed Priced Award in accordance with the following: Set Aside: This acquisition is a 100% Women Owned Small Business set-aside. The Product Service Code is Q602, Medical Records. The North American Industrial Classification System (NAICS) code for this acquisition is 541611 with a size standard of $16,500,000.00. The service shall be provided in accordance with (IAW) the attached Solicitation and Performance Work Statement (PWS). The period of performance for the service is one (1) Base Year with three (3) one-year options: Base year from September 1, 2022 to May 31, 2023, Option Year 1 from June 1, 2023 to May 31, 2024, Option Year 2 from June 1, 2024 to May 31, 2025 and, Option Year 3 from June 1, 2025 to May 31, 2026.
