The USPFO for Puerto Rico, located at Fort Buchanan, Puerto Rico, intends to issue a Request for Quote (RFQ) to award a single firm fixed-price contract for Dentistry Services in Puerto Rico northern area for a base period of seven (7) months and four option years. The evaluation factor will be de lowest price. The contractor shall provide all personnel, equipment, facility, tools, materials, supervision, and quality control necessary to perform Dentistry Services as defined in the Performance Work Statement (PWS). The contractor shall provide services between normal operation hours from Monday through Sunday. Performance shall be in Puerto Rico northern area. The North American Industry Classification System (NAICS) code for this work is 621210. This is a total small business set aside. The small business size standard is $8 million average annual revenue for the previous three years. Period of Performance (PoP). The Period of Performance shall be one Base period of 7 months and 4 / 12-month option years. The period of performance for the base period is: September 2021 to April 2022. The period of performance for Option Year 1: May 2022 to April 2023. The period of performance for Option Year 2: May 2023 to April 2024. The period of performance for Option Year 3: May 2024 to April 2025. The period of performance for Option Year 4: May 2025 to April 2026 The solicitation closing date is scheduled for 13 September 2021 at 2:00 P.M.
