The Puerto Rico Army National Guard has a requirement to provide Medical and Dental Non-clinical Case Management and administrative Care Coordinator staffing in support of Individual Medical Readiness (IMR) for the Puerto Rico Army National Guard as described in the Performance Work Statement (PWS)(Attachment 1). The case management services shall maintain communication as required with the Service Member; all shall be documented in the Medical Readiness Portal and MEDCHARDT Applications to include Medical Operational Data System (MODS). The Contractor shall serve as liaison between the chain of command and the chain of concern, providing assistance to Soldiers with all aspects of medical readiness and mobilization to include Periodic Health Assessments and individual Soldiers readiness to determine deployability status of the Individual Soldier. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only notice. A written solicitation will not be issued. This requirement is issued as a request for quote (RFQ). The solicitation documents and incorporated provisions and clauses are those in effect in the Federal Acquisition Circular 2005-95. A Firm Fixed Priced Award will be made in accordance with the following: NAICS Code: This acquisition is being solicited as a 100% set aside for small business -Competitive (8a).
