Amendment 0001 dated 16 February 2021. The purpose of this amendment is to extend the date and time for receipt of quotes to Friday, February 19, 2021 at 1:00p.m. ET. All other terms and conditions remain unchanged. This solicitation is being issued as a combined synopsis/solicitation for commercial items, prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement and corresponding attachments constitute the only solicitation; quotes are being requested and a separate solicitation will not be issued. Solicitation W912LQ-21-Q-0006 is being issued as a Total Small Business Set-A-Side. The issuance of this requirement is a Request for Quote (RFQ) as an Indefinite Delivery, Indefinite Quantity (IDIQ) contract. Only one awardee will be chosen for this IDIQ contract. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2021-04, effective 19 January 2021. This procurement is being issued as full and open competition under NAICS code 621111; Offices of Physicians (except Mental Health Specialists) and PSC Q403; Medical – Evaluating/Screening with a business size standard of $12,000,000. The Virginia Army National Guard has a requirement for Occupational Health Medical Surveillance that will be mobile to travel up to five (5) different locations. The medical surveillance services includes blood work, vital signs, and physician examination and documentation management. Exams are to be completed in accordance with 29 Code of Federal Regulations (CFR) 1910- Occupational Safety and Health Standards for General Industry; January 2018, 5 CFR 339.205. Medical Evaluations Program; January 2018, Army Regulation (AR) 40-5, Preventive Medicine; May 25, 2007, and the Virginia Army National Guard SOP Medical Surveillance, January 25, 2019.
