This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, W912LP19R0036, is being issued as a Request for Proposal (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2019-01, effective 22 Jan 2019. This procurement is a total small business set-aside. The NAICS code is 621511 and the small business size standard is $32,500.00 million. Basis for award is Lowest Priced Technically Acceptable. The following commercial services are requested in this solicitation: The contractor shall provide to the 250 PR ARNG Technician Employees the Annual Surveillance Physical Exams who fall under the PRANG Occupational Health Program. These medical exams will determine any employee's work-related injury/illness or any hazardous exposure and note any finding of employee medical/work limitation. The medical surveillance exam reports once reviewed will give the employee medical determination/recommendations or clearance for the duties outlined in the provided job description (PD document). The contractor/company shall provide all: labor, material, equipment, certifications, and supplies necessary to provide Annual Surveillance Physical Exams services. These services will complete physical exams, proper documentation for each Technician Employee, as necessary to meet the NGB DoD 6055 .05-M, May 2, 2007, Occupational Health Examination and Surveillance Program and DoD Directive 4715.1 E, March 19, 2005, Environment, Safety, and Occupational Health (ESOH). Period of Performance (PoP). 21-23 May 2019 Service Contract Labor Standards (formerly Service Contract Act) applies to this acquisition. For applicable Wage Determination, refer to FAR clause 52.222-49, Service Contract Labor Standards -- Place of Performance Salinas, P.R, within Attachment #2. Respond by uploading completed solicitation signed with offered prices in the enclosed SF-1449 into this notice. Please utilize for this purpose table in Section B (Tests/Exams-For Pricing). All vendors must include in block 17a of the solicitation your contact information, cage code, name of company, address, phone number, and email address also ensure to complete block 30 (all letters). Please write the total amount for each one of the contract line items for your bid on the solicitation (SF-1449) and include a grand total for all contract line items as well at the bottom. Offers not uploaded and uploaded into this notice not containing the aforementioned information shall not be considered. In the event of technical difficulties are experienced, alternatively and with prior notice to the P&C office point of contact, email submittals will be accepted and considered for evaluation. In accordance with AR 530-1, operations security, new contractor employees shall complete level I OPSEC training within 10 calendar days of their reporting for duty and annually thereafter. The contractor shall submit certificates of completion for each affected contractor employee, to the POC, or to the KO if a POC is not assigned, within 10 CALENDAR DAYS AFTER COMPLETION OF TRAINING. Questions and Answers (Q&A) will be consolidated and posted to FedBizOpps on a non-attribution basis. The final Q&A will be posted no later than 3:00 p.m. on 2 May 2019. Proposals are due by 11:00 a.m. local (eastern) time on 6 May 2019. Regardless of how proposals are sent, the contractor is responsible for ensuring that they are received in the USPFO-PR P&C Office by the due date/time.
