This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation, W912LP-22-Q-4512, is being issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2022-07, effective 08/10/2022. This procurement is being solicited for full and open competition. The NAICS code is 62111 and the small business size standard is $12.0 million. The following commercial services are requested in this solicitation: The Contractor shall provide occupational health examinations for 143 Iowa Army National Guard full-time support and/or temporary personnel to include, vital signs, vision screenings venipunctures, labs and physical/dermatological exams, in accordance with the Performance Work Statement (PWS). Estimated Period of Performance: Within 60 days after contract award. Contract Line Item Numbers (CLINs), Quantities and Units of Issue are as follows: CLIN 0001. Medical Exam, Quantity/Unit of Issue: 116 Each CLIN 0002. Vision Screening, Quantity/Unit of Issue: 116 Each CLIN 0003. Complete Law Draw, Quantity/Unit of Issue: 75 Each CLIN 0004. Lead Lab, Quantity/Unit of Issue: 45 Each CLIN 0005. Chromium Lab, Quantity/Unit of Issue: 10 Each CLIN 0006. Manganese Lab, Quantity/Unit of Issue: 8 Each CLIN 0007. Cadmium Lab, Quantity/Unit of Issue: 75 Each Proposed prices shall be firm fixed price (FFP) and inclusive of all labor, materials, lab fees, and associated services required to provide the medical surveillance examinations as described within the PWS. Service Contract Labor Standards (formerly Service Contract Act) applies to this acquisition. See Attachments 4 and 5 for the applicable Wage Determinations. Failure to complete and submit Attachment 3 may result in your offer being considered non-compliant and it may not be considered for award.
