This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 1) The Indiana Army National Guard is soliciting W912L9-23-Q-0006 as an RFQ 2) This Firm Fixed Price (FFP) procurement is a 100% Small Business set-aside in accordance with (IAW) the Federal Acquisition Regulation (FAR) Part 19.502-2 (b). 3) The CLINs listed below (5) identify the Brand Name (or Equal) Specifications the requiring activity is looking for. 4) Please price any associated shipping/installation/removal costs within the Item CLIN and not on a separate clin. 4a) Door to Door delivery is preferred, but dock shipments would be acceptable, with follow on coordination for assembly and set up at the final destination. The Gov't has the personnel to move unassembled, boxed treadmills into the rooms where they will be assembled and set-up by the vendor. 5) CLIN 0001 - Qty. - 11 - Life Fitness Deluxe Integrity Series Treadmill with SL Console. (includes shipping, delivery and setup) 5a) Minimum Salient Characteristics shall meet the following requirements: Digital Console with function adjustments and wireless connectivity. Operates at speeds between .5 and 14 mph. Incline adjustments between 0% and 15%. Handrails no less than 16" long. Run surface no less than 22" wide by 60" long. Supports a user weight of up to 400 lbs. Frame and Motor warranty shall be no less than 7 years. 6. Quotes shall include the following: Company Name Street Address City, State, and Zip Remittance Address DUNS number CAGE Code UEI Tax ID Discount Terms A technical description in sufficient detail to evaluate similarity with the specifications associated with the Name Brand/Model Number listed above. Clauses 52.204-24 and 52.204-26 must be reviewed and checked accordingly. 7. The Government will award a contract resulting from this RFQ to the responsible offeror whose offer conforming to the RFQ will represent the BEST VALUE to the Government. 8.
