ANSWERS TO RFI POSTED REVISED PWS POSTED QUOTE DUE DATE REMAINS UNCHANGED This is a non-personnel services contract to provide Mobile Medical Surveillance Physicals.This is a non-personnel services contract to provide Mobile Medical Surveillance Physicals. The contractor shall provide all personnel, equipment, tools, materials, supervision, and quality control necessary, except as specified as Government Furnished, to perform Medical Surveillance Physicals for approximately 70 Wyoming Army National Guard Federal Technicians. Selected full-time Federal Technicians that work for the Wyoming Military Department are required to receive annual medical examinations in accordance with DoD Instruction 6055.5, DoD Instruction 6055.5-M, and OSHA 29 CFR 1910. The Period of Performance shall be one-Base Year and four 12-month option years. The contractor shall develop and maintain an effective QC Plan to ensure services are performed in accordance with this PWS. The contractor shall develop and implement procedures to identify, prevent, and ensure non- recurrence of defective services. The U.S. Government or State Occupational Health Nurse will evaluate the contractor’s performance under this contract in accordance with the Quality Assurance Surveillance Plan. The primary work site is the Army Aviation Support Facility (AASF) Complex 7600 Rogers Drive in Cheyenne, WY 82005. (See attached Performance Work Statement for detailed description of services.)
