The purpose of this requirement is to provide full time medical examination services in support of individual Soldier medical readiness as prescribed by AR40-501. Services shall also include completion of periodic health assessment (PHA), pre/post deployment health assessment (PDHA), post deployment health reassessment (PDHRA), Physical Examinations for Aviation, Special Military School and miscellaneous examinations to improve Individual Medial Readiness (IMR) of the TXARNG. Place and Performance of Services: The contractor shall provide services between the hours 0800-1630 Monday – Friday with occasional work on Saturday and Sunday on an as needed basis at the following locations: San Antonio Camp Bullis, 21200 Camp Bullis Road, Bldg 5044, San Antonio, TX 78257, Austin Camp Mabry, 2200 W. 35th Street, Bldg 86, Austin, TX 78730, Ellington Field Medical Command Medical Clinic, 1182 Ellington Field, Houston, TX 77034, and Grand Prairie Medical Command Medical Clinic, 1013 Lakecrest Drive, Grand Prairie, TX 750813.
