This is a sources sought for the purpose of supporting the Kentucky Army National Guard (KYARNG) Occupational Medical Surveillance Examination Program is to provide job-related medical surveillance examinations and occupational health services to approximately 330 KYARNG full-time support (FTS) or temporary civilian federal personnel who are exposed or potentially exposed to health hazards in the work environment, or who are assigned to positions requiring specific standards of physical fitness. Place and Performance of Service: To conduct medical surveillance examination/occupational health services in the following locations listed below. Proposed start date of 1 October 2021. Lexington, Kentucky Area – approximately 170 personnel Elizabethtown, Kentucky Area – approximately 100 personnel Greenville/Owensboro, Kentucky Area – approximately 60 personnel TOTAL: approximately 330 personnel/Year Medical Examinations and Surveillance Provide/scheduling the following exams: Initial Hire Exam(based on site environment and duty position) Annual Exam(based on site environment and duty position) Termination Exam(based on site environment and duty position) Conducted exams during Initial/Annual/Termination Exam will be tailored to the individual base on work site and duty position hazards and exposures as prescribed by a State Occupational Health survey/worksheet. All Examinations defined: Complete Physical Exam/Fit for Duty to include: Vitals Ability/Proper lifting techniques up to 50 lbs. Range of Motion Eye Exam -Near/Far -Depth -Color -Peripheral -Corrected/Uncorrected Urine Analysis Lipid Panel Comprehensive Metabolic Panel (CMP) Complete Blood Count (CBC) w/Differential Venipuncture The following exams based upon age (40 and above) and occupational exposures as identified: Zinc Protoporphyrin (ZPP) Liver Panel Spirometry Chest X-Ray (Posterior Anterior) Chest X-Ray B-reader Electrocardiography (EKG/ECG) Cadmium in Blood Cadmium in Urine ?eta-2 Microglobulin in Urine PROCEDURES AND STANDARDS: Providers must ensure all procedures are conducted in accordance with (IAW), if applicable, American Board of Preventive Medicine (ABPM), American Association of Occupational Health Nurses, (AAOHN), U.S. Department of Labor, Occupational Safety and Health Administration (OSHA), and National Institute for Occupational Safety and Health (NIOSH) Standards. Providers must ensure all clinical laboratory services are performed by a laboratory licensed through the U.S. Department of Health and Human Services (DHHS) Health Care Financing Administration pursuant to the terms of the Clinical Laboratories Improvement Act of 1967 (42 U.S.C. 263a) and the college of American Pathologists. Providers must provide a Board Certified Occupational Medicine Physician(s) to conduct job-related medical surveillance physical, license must be available. Examinations and occupational health consultation services are for approximately 330 KYARNG employees. Providers will arrange for initial coordination of services with the Project Officer within fifteen (15) working days after award of the contract. Providers will arrange for the opportunity to consult with a representative of the KYARNG by telephone to coordinate or resolve issues that might arise before, during, and after the testing period. A comprehensive review and evaluation will be performed within thirty (30) days following the conclusion of testing. The KYARNG will contact and schedule a follow-up review and evaluation meeting to discuss positive and negative aspects of testing. Only licensed medical technicians will perform medical surveillance screening/testing elements. Personnel performing audiograms will be certified by the Council for Accreditation in Occupational Hearing Conservation (CAOHC). Personnel conducting spirometry will be certified through a program approved by the national Institute for Occupational Safety and Health (NIOSH). License and certifications must be available. SAFETY: Work will be performed in the provider’s facilities: therefore safety issues are within a normal scope of operations. MEDICAL REPORTS AND RECORDKEEPING: The following forms are mandatory in the inclusion of medical reports and records, and will be provided by KYARNG personnel: Optional Form 178, Certificate of Medical Examination DD Form 2807-1, Report of Medical History OSHA Respirator Medical Evaluation Questionnaire Appendix C to Sec. 1910.134 (if identified for respirator clearance.) Providers will ensure two (2) copies of the employee examination findings are furnished. One copy is to be sent to the KYARNG G1 Health Services: Each KYARNG personnel will sign a release of medical examination results, which shall include all diagnostic tests, graphs, charts, reports, questionnaires, waivers, employee consent forms, physical findings, and examination results. One copy is to be mailed directly to the individual employee’s home of record and one to the KYARNG G1 Health Services. The contractor will provide the KYARNG Health Services with a hardcopy of these results within thirty (30) working days of completion of testing. Results of medical surveillance testing which are questionable and/or of critical significance will be communicated directly, and immediately, to the KYARNG Project Officer by telephone, email, and/or express mail. At a minimum, all medical records are to be identified by the contractor with the employee’s name, social security number, gender, job code, and the name of the employee’s workplace or facility. The contractor must ensure work/health history forms and medical reports are complete and legible (typed/printed). With respect to supplemental forms, Employees will be instructed to complete a release form if required by the contractor. The Project Officer will provide the contractor with a Privacy Act Statement (DD Form 2005) for each employee to sign during the registration process. Disposition of Records: In maintaining compliance with HIPAA Privacy Rule and the HIPPA Security Rule all information, records, and data will be protected with appropriate safeguards in place. The HIPPA Privacy Rule and HIPPA Security Rules applies for the entire retention period until the records have been properly destroyed. Provider will follow State Law for retention period for medical records. HIPAA Privacy Rule and HIPAA Security Rule defines the requirement for destruction of all medical and personal health records in paper and electronic media and shall be followed to maintain protection of KYARNG employees information. At no time shall any KYARNG employee’s information, personal or medical, be shared with any unauthorized individuals, companies, or entities to which a violation of HIPAA will take place. EXAMINATION SCHEDULING Coordination of scheduling will be handled by event schedulers during normal duty day/hour within the performance period dates defined. Employees will be given adequate time for travel to and from exam site, time to complete examination, and attend any follow-up requirements at request of examiner. Initial examinations will be conducted, with every effort, on individuals identified by KYARNG Human Resource Office prior to coming on board as a KYARNG civilian federal employee, and scheduled by event schedulers. Initial examinations may be conducted after the individual has come on board and is in a civilian federal employee regular hour pay status. At no time will personnel have examinations conducted in Active Duty, Active Guard/Reserve (AGR), Drill, or any other military pay status. Annual and termination examinations will be conducted while KYARNG personnel are in a civilian federal employee regular hour pay status or earning compensatory time. No personnel in Active Duty, Active Guard/Reserve (AGR), Drill, or any other military pay status will be allowed to be scheduled for annual or termination examination until a return to civilian federal employee pay status has been made. Missed appointments will be handled between local KYARNG supervisor and exam site. KYARNG personnel will make every attempt to notify exam site and supervisor of cancellation or re-scheduling as soon as possible. In the event that exam site cannot operate during a scheduled period the exam site shall notify both the personnel scheduled for examination and the KYARNG supervisor, of the scheduled personnel, as soon as possible and work to re-schedule examination.
