The purpose of this requirement is to provide licensed practical nurse (LPN) services in support of individual Soldier medical readiness as prescribed by AR 40-501. Every Soldier requires anthropomorphic measurements, vital signs, visual acuity, phlebotomy, audiology and a small subset electrocardiograms and basic point of care laboratory testing on an annual basis. Medical treatment is not authorized. Period of Performance (PoP). The Period of Performance shall be 1 Base Year of 12 months plus one option year. Place and Performance of Services. The contractor shall provide a licensed practical nurse between the hours of 0630-1700 on Tuesday through Friday, except on recognized US holidays or when the Government facility/installation is closed due to local or national emergencies, administrative closings, or similar Government-directed facility/installation closings. Performance shall be at Camp Fretterd Military Reservation (CFMR), 5555 Rue Saint Lo Drive, BLDG 205, Reisterstown Maryland, 21136 and at The Flight Surgeon’s Clinic 2624 Fairview Point Road, Building E-6814, Edgewood Maryland 21040.
