Which method is being used here? Answer: Tradeoffs are being used where all evaluation factors other than cost or price when combined are approximately equal to cost or price. 2. Question: Ref: PWS: Is Nursing Malpractice insurance required? Answer: No, LPNs do not require malpractice insurance. 3. Question: Ref: Q&A: Should the proposal be submitted in one complete document or broken down into separate documents based on technical response, past performance, and pricing? Answer: Only the pricing submission must be in a separate document. See the basic format in the RFQ. 4. Question: PWS 1.4.4 states 32 hour weeks for a 1664 hour year. PWS 2.1.13 suggests a 10hr x 4 day work week which is a 2080 hour year. Please confirm we should bid 32-hour (8hr x 4 day) work weeks. Answer: Yes, use the 32-hour (8hr x 4 day) work weeks for your pricing calculation. 5. Question: The RFQ Amendment 1, includes this statement: “The MDARNG Surgeon’s Office requires a LPN(s) to perform approximately 1664 hours of dental examination services per year. On average one LPN will be required per day.” Please clarify: should the word “dental” be included in this requirement? Answer: This is an editing error, replace “dental examination” with “LPN services”. See PWS Paragraphs 1.2 and 5.0 for details. Question: I was putting together the price quote for this solicitation and noticed the format contained a Contract Manpower Reporting Application (CMRA) at 1 each. Can you please assist me on what information needs to be completed for that portion? or will that portion be n/a? Answer: You should re-read the RFQ for the eCMRA website to understand the reporting requirement. Pricing decisions are solely offeror decisions. 2. Question: I’m preparing a response to the sources sought for a licensed practical nurse. In the evaluation factors document, under 3., it states “Offerors shall provide pricing on a separate page in the format used in the RFQ in the response to this solicitation to include monthly prices and total price.” I’m unable to find the format. Question: I'm also having a really difficult time finding the sources sought documents (they were sent to me). On the FBO page, it indicates that there are not any attachments. (end of Amendment 1) The Maryland Army National Guard requires NON-personal services to provide timely and accurate Licensed Practical Nurse Services. The purpose of this contract is to provide Licensed Practical Nurse (LPN) services in support of Individual Medical Readiness (IMR). We propose to enter into a Firm-fixed Price contract with a contractor who shall provide LPN Services as defined in the attached Performance Work Statement (PWS). The Government is seeking a contractor having LPN Services expertise to provide nursing services to include; anthropomorphic measurements, vital signs, visual acuity, phlebotomy services, audiology testing, and a small subset electrocardiograms and basic point of care laboratory testing for soldiers being tested on an annual basis. The MDARNG Surgeon’s Office requires a LPN(s) to perform approximately 1664 hours of dental examination services per year. On average one LPN will be required per day. Offerors shall assume that the Government has no prior knowledge of their experience, and will base its evaluation on the information presented in the Offeror’s proposal. The proposal should not simply rephrase or restate the Government’s requirements, but rather shall provide convincing rationale to address how the Offeror intends to meet these requirements. A candidate Professional resume(s) must be provided. Capability and Experience: Proposal should demonstrate an understanding of the requirements as outlined in the Performance Work Statement. The result of this market research will contribute to determining the method of procurement, to include the use of any Small Business Set-aside opportunities exist, and which Small Business Set-aside categories should be considered.
