This is a solicitation for commercial items prepared in accordance with the format in FAR part 13, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number W912J2-20-R-0007 is being issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-78. This requirement is 100% set-aside for Small Business concerns under North American Industry Classification System (NAICS) code 621399 - Offices of All Other Miscellaneous Health Practitioners with a Small Business size standard of $7.5 million dollars. The requirements of this contract include providing all qualified personnel, materials, supervision, and quality control necessary, to perform Technician Employee Occupational Health Exam Services in accordance with the attached Performance Work Statement for the Wisconsin Army National Guard. The Period of Performance shall be one Base Year of 12 months beginning 1 July 2020 and (three) 12-month option years. Performance shall be at established permanent building clinics within the state of Wisconsin, within 75 miles of the following cities: Madison, Milwaukee, Eau Claire, Wausau, Oshkosh, Kenosha, Tomah, Wisconsin Rapids, and Hayward.
