This announcement constitutes the only solicitation; quotations are being requested on the provided SF 1449 and pricing sheet(s). This solicitation is issued as a request for quotation (RFQ) in accordance with FAR Part 12 and 13. All responsible source(s) solicited may submit a quote that shall be considered. Interested parties may identify their interest and capability to respond to the requirement or submit a quote. Offerors must complete and submit the SF1449 Solicitation form. Offerors must submit the provided “Dental Treatment Pricing Sheet” for either the Reno/Carson City Area or Las Vegas Area or both. Offerors must also specify the name and location of the Dental Office where services will be performed. Past performance will be requested if the Government considers it necessary during evaluation of offers.
