This is a Firm Fixed Price Request for Quotation (RFQ) being issued by the Nevada Army National Guard (NVARNG) United States Property and Fiscal Office Purchasing and Contracting Division for Occupational Health Examination services. The anticipated period of performance is 01 July 2022 – 30 June 2023 at contractor specified facilities/ locations in the greater areas of Reno and Las Vegas, Nevada. To clarify, offerors must have at least one facility in the greater Reno area and at least one facility in the greater Las Vegas area that are equiped and purposed for conducting Occupational Health Physical Examinations. This solicitation is prepared in accordance with FAR Part 12, the acquisition of commercial items in conjunction with the policies and procedures for solicitation, evaluation, and award as prescribed in FAR 13, Simplified Acquisition Procedures. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2022-06 dated 5/01/2022. This solicitation is a TOTAL SMALL BUSINESS SET-ASIDE and the North American Industry Classification System (NAICS) code for this solicitation is 621999 with an industry size standard of $16.5 million and the Product Service Code (PSC) is Q403. A responsible offeror/quoter is one whose offer conforms to the solicitation and will be most advantageous to the Government. Please refer to 52.212-2 Evaluation - Commercial Products and Commercial Services (page 8 of the attached SF1449). Quoted pricing on the SF1449 must be completed and submitted. The SF1449 provides the contract clauses, terms and conditions for this requirement. Questions concerning this requirement shall be addressed in writing to Greg Kothman at gregory.p.kothman.civ@army.mil prior to 11:59pm Pacific Time on Monday, May 23, 2022. This is the only means of submitting questions. All questions will be consolidated and answered in the form of a solicitation amendment and posted to this site. Quoted pricing and any related documentation required for evaluation (SF1449 page 3 for pricing, and other documentation is listed on page 8 under 52.212-2 Evaluation) shall be submitted to Greg Kothman at gregory.p.kothman.civ@army.mil prior to 11:59 PM Pacific Time on Monday, June 13, 2022. This is the only means of submitting quotes in response to this RFQ.
