This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a Firm Fixed Price Request for Quotation (RFQ) being issued by the Nevada Army National Guard (NVARNG) United States Property and Fiscal Office Purchasing and Contracting Division for Medical Readiness in Support of Individual Medical Readiness (IMR) and Fully Medically Ready (FMR) event 11 - 12 January 2020 at NVARNG locations in Reno and Las Vegas, Nevada. This solicitation is prepared in accordance with FAR Part 12, the acquisition of commercial items in conjunction with the policies and procedures for solicitation, evaluation, and award as prescribed in FAR 13, Simplified Acquisition Procedures. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2019-06. This solicitation is a TOTAL SMALL BUSINESS SET-ASIDE and the North American Industry Classification System (NAICS) code for this solicitation is 621498 with an industry size standard of $22 million and the Product Service Code (PSC) is Q403. A responsible offeror / quoter is one whose offer conforms to the solicitation and will be most advantageous to the Government. All firms or individuals responding must meet all standards required to conduct business with the Government, including registration with the System for Award Management (SAM), and be in good standing. All qualified responses will be considered by the Government. In addition to providing pricing each quote must include vendor's CAGE/DUNS number, and must include point of contact information. Failure to provide SAM registration may result in your quote being disqualified from consideration. IAW FAR 52.204-7 and DFARS 252.204-7004 Alt A, prospective vendors must be registered in the SAM database prior to award. Prospective vendors should visit the SAM website at https://sam.gov/SAM/pages/public/index.jsf to register. Quoted pricing can be submitted via company formatting, or the SF1449 can be completed and submitted as the price quote. Nevada Army National Guard (NVARNG) United States Property and Fiscal Office Purchasing and Contracting Office intends to issue a Firm-Fixed-Price Request for Quotation (RFQ) for Physical Health Assessment (PHA) Services in support of Individual Medical Readiness (IMR) and Fully Medically Ready (FMR). The line item descriptions are as follows: Item 0001: The Contractor shall provide PHA event services at the NVARNG Plumb Lane Armory located at 685 East Plumb Lane, Reno, NV 89502 in accordance to the Draft Performance Work Statement (PWS). The services shall include one (1) Flight Surgeon. Item 0002: The Contractor shall provide PHA event services at the NVARNG Las Vegas Readiness Center located at 4500 West Silverado Ranch Blvd., Las Vegas, NV 89139 in accordance to the Draft Performance Work Statement (PWS). The services shall include two (2) Registered Nurses; two (2) Behavorial Health Specialistsand one (1) Flight Surgeon. Item 0003: The Contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Nevada Army National Guard via a secure data collection site. The Unit Identification Code (UIC) for the Requiring Activity is W90092, and the Issuing Office DoDAAC is W9124X. The contractor shall completely fill in all required data fields using the following web address: http://www.ecmra.mil/. Reporting inputs shall be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year, beginning with 2020. The requirement is not currently funded contingent upon passing of the Continuing Resolution.
