This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, W91243-21-Q-TB05, is being issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2021-06, effective 7/12/2021. This procurement is a 100% Small Business set-aside in accordance with (IAW) the Federal Acquisition Regulation (FAR) Part 19.502-2 (b). The following commercial services are requested in this solicitation: Evaluation for Award shall be based on price and nonprice related factors that consist of documented evidence rpovided with the quote that the offeror can comply with the PWS as written. Contractor shall provide job-related Medical Surveillance Examinations and Occupation Health Services to the NEARNG full-time support (FTS) and/or temporary personnel who are or could potentially be exposed to health hazards in the work environment, in accordance with the attached Performance Work Statement (PWS).
