All details regarding the requirement are clearly listed on the attached specifications. This requirement is strictly for (1) BRAND NEW ITEMS, not refurbished nor resales, and (2) items must be a 100% match to the stated Specifications (no exceptions). Therefore, All bidders must comply with the requirements stated on the attached Specifications & Requirements document. Bidders cannot deviate from the stated requirements with alternate items that do not specifically match what’s required. Failure to meet the requirements or not to comply with what is specifically requested will be considered as “Non-Responsive”. All bidders must complete and sign / date the Solicitation (SF1449) with all quoted offers. We will not be able to accept a quote unless we receive a signed / dated SF1449 to go with the quote. All bidders must submit the following documents with their bids: 1. SF1449 (filled in, completed, and signed as specifically instructed below) 2. Company Quote (must be FOB Destination; see Note 3 below) Two attachments of the SF1449 are included for your use (PDF & WORD). In case if one doesn’t work, use the other. You only need to complete ONE as follows… PAGE 1: Block 24 (Combined Overall Total for the entire Contract) PAGE 1: Block 30a, b, & c (must be *SIGNED/DATED [not typed]; can't accept w/o signature) *NOTE: Digital Card signature acceptable.
