The performance period for Facility CO128 is from 1-December-2020 to 30-November-2021 and Facility CO130 is from 15-December-2020 to 30-November-2021, including four (4) option years and a six (6) month option to extend services. Offeror shall submit a signed SF 1449, completed information from the Supplemental section and a completed Price Schedule (Attachment 1).
