Cost of service to include: Two (2) on-site visits per month to review prescriptions, order safety glasses, dispense, fit, adjust glasses, and attach safety side shields as needed. It is anticipated that 3 to 6 employees will be serviced with glasses per visit. Watervliet arsenal will provide contractor a table & 2 chairs in a room that can be locked and used each visit. NYS licensed Ophthalmic dispenser is requirement for successful offer.
