This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested. This solicitation is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-06; 12 July 2021. This acquisition is brand name or equal and total small business set-aside solicitation. The North American Industry Classification System (NAICS) Code is 339113 – Surgical Appliance and Supplies Manufacturing with a size standard of 750 employees. The basis of award for this acquisition will be made using the Lowest Price Technically Acceptable (LPTA) selection procedures. Reference attached RFQ for detailed information on the requirement, how to respond, and evaluation criteria. The subject header of the e-mail submission of questions shall include the RFQ number W81XWH-21-Q-0234. Questions received after this date will not be accepted.
