The purpose of this amendment is to correct/include the following:: 1. Attachment 4, Staffing Chart, Productive Hours on year 1, from 1920 to 1760. 2. Correct Performance Work Statement (PWS) reference in the Addendum to 52.212-1 Table 1.k-Proposal Organization Chart, Contractor Furnished Supplies/Property Requirements from 4.3.3 and 4.3.14 to 4.3.13. The PWS reference has been revised throughout the Addendum to 52.212-1 and is highlighted in red. 3. Delete linen references on the Quality Assurance Surveillance Plan, paragraphs 7c. and 7d. (Revision 1) 4. Submit a revised Attachment 3, Price Matrix (Revision 1). The revision corrects formula errors throughout. 5. Delete the following sentence from the Performance Requirements Summary, paragraph 7.1, “The Contractor shall be required to correct/eliminate any defects in Type I or II Services within 30 minutes from receipt of Inspection Discrepancy Notification or Customer Complaint Record.” 6. Include pictures of some buildings and flooring types throughout Medical Treatment Facilities in the health service area of Kenner Army Health Clinic, Fort Lee, VA. 7.
