The purpose of this amendment is to correct the square footage total on Exhibit D, cell Q4646 from 81,337 to 81,724. The previous amendment 0006 stated that the total square footage decreased from 1,367,990 to 1,367,605. A re-calculation of the total square footage has been computed at 1,367,990. As a result, Exhibit C, - Summary of Bldg. Sq. Ft/Type Service (New -Revision 3), Exhibit D-Detail, Building/Rooms/Sq Ft/Type Service (New -Revision 3), Attachment 3, Price Matrix (New -Revision 6), Attachment 4, Staffing Charts (New -Revision 4) and Performance Work Statement paragraph 1.4 have all been corrected to reflect the total square footage of 1,367,990. The purpose of this amendment is to correct square footage subtotals for contract line item numbers, 0006 (Bldg.18511E), 0007 (Bldg.18511W), 0009 (Bldg. 1026), 0014 (Bldg. 11282), 0017 (Bldg. 11335) and 0018 (Bldg. 1363), which then resulted in a change (decrease) to the overal square footage from 1,367,990 to 1,367,605. The updated Attachment 3, Price Matrix revises the max SF for Bldg. 18511E from 196,423 to 203,191. All other terms and conditions remain the same. Amendment 0004 - See attached amendment. The proposal due date of 3 December 2021, 12:00 p.m. Central Standard Time remains. The purpose of this amendment is to correct formula errors on Attachment 3, Price Matrix. The updated Attachment 3, Price Matrix is now Revision 3. CLIN 0007, Bldg 18511W – West, Max SF cells G10, G19, G28, and G37 have been corrected to indicate max SF of 222,663. CLIN 0038, Bldg L10735 – Sum of Unit Prices for cells D27, D36, and D45 have been corrected so that it sums all of the applicable fields. All other terms and conditions remain the same. The Health Readiness Contracting Office, Joint Base San Antonio, Fort Sam Houston, TX intends to award a Single-Award Indefinite-Delivery, Indefinite-Quantity contract with firm-fixed priced task orders on behalf of William Beaumont Army Medical Center, Fort Bliss, Texas for healthcare environmental cleaning and related services. The services have been determined to be commercial in nature. The NAICS code for this acquisition is 561720-Janitorial Services and the small business size standard is $19.5 million. This requirement will be awarded using competitive procedures and will be procured as a 100% Set-Aside for small business concerns.
