This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Medical Readiness Contracting Office - Pacific, Bldg 339 Clark Road, Ft Shafter, HI 96858, intends to solicit, negotiate, and award a contract with Firm Fixed Prices for Lin-Zhi 6-ACETYLMORPHINE(6AM) Item 0291 for Tripler Army Medical Center (TAMC), Hawaii. The solicitation number for this request for quotation (RFQ) is W81K0223Q0077. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2023-01. The Standard Industrial Classification Code is 6515 and the NAICS code of 325413. This procurement is unrestricted BRAND NAME ONLY. Contractors shall submit a quote on the following (see Attachment 1): CLINs 0001 through 4001. Period of Performance 01 Sep 2023 through 30 September 2027. The provision at 52.212-1, Instructions to Offerors -- Commercial, and its addendum applies to this acquisition. See Attachment 1. The provision at 52.212-2, Evaluation - Commercial, and its addendum applies to this acquisition. See Attachment 1. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, and its addendum applies to this acquisition. See Attachment 1. The deviation to clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. See Attachment 1 for the FAR clauses that are applicable to this acquisition. The Defense Priorities and Allocations System (DPAS) and assigned rating is not applicable. The Government intends to award a contract with Firm Fixed Prices for a base period of 01 Sep 2023 through 30 Sep 2023 with four (4) one-year options to the successful quoter. Contract Requirements - The successful offeror shall be required to meet the requirements of the combined synopsis/solicitation. Contractors shall quote their unit pricing based on the quantities identified in each Contract Line Item Number. See Attachment 1 for Proposal Instructions / Evaluation Criteria.
