This amendment is to provide information regarding a scheduled site visit and answer vendor questions received for W81K0222Q0083 Combined Synopsis/Solicitation. Reference the attached file – Amendment 3 Site Visit and Vendor Questions. The closing date remains unchanged 8 August 2022 and all other terms and conditions remain unchanged. This combined synopsis/solicitation amendment is to update the PWS in its entirety and update the minimum requirements in W81K0222Q0083 Attachment 1 Revised. Please provide quotes utilizing this revised PWS in W81K0222Q0083 Attachment 1 Revised. Due to email account technical difficulties, interested parties must resend all submissions, quotes, and email correspondence that was sent during the periods of June 21, 2022 to July 13, 2022. The Government is not responsible for any email correspondence sent during the period of June 21, 2022 to July 13, 2022. The closing date remains unchanged 8 August 2022. This combined synopsis/solicitation has been amended to extend to close date to 8 August 2022. All other terms and conditions remain the same. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Regional Health Contracting Office - Pacific, Bldg 339 Clark Road, Ft Shafter, HI 96858, intends to solicit, negotiate, and award an Indefinite Delivery Indefinite Quantity Contract with Firm Fixed Prices for Medical Clinical Gas and Delivery Services for Osan Air Base, Korea. The solicitation number for this request for quotation (RFQ) is W81K0222Q0083. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-06. The Standard Industrial Classification Code is 6515, size standard is 1000 employees and the NAICS code of 325120. This procurement is unrestricted. The Government intends to award an Indefinite Delivery Indefinite Quantity contract with Firm Fixed Prices contract for a base period of 25 Oct 2022 through 30 September 2023 with four (4) one-year options to the successful quoter.
