This announcement constitutes the only solicitation; request for quotes are being requested and a written solicitation will not be issued. This solicitation W81K0220Q0061 of Purchase Request BPAMAMC2020-1 is issued as a Request for Quote (RFQ). Regional Health Contracting Office-Pacific, Joint Base Lewis-McChord (JBLM, Health Contracting Branch Lacey WA; intends to award multiple Blanket Purchase Agreements (BPAs) for FDA Approved Blood and Blood Products to include but not limited to; Whole Blood, Red Blood Cells, Platelets, Cryo, and related Blood Bank supplies for Madigan Army Medical Center, Tacoma WA; Medical Laboratory, Blood Bank, Transfusion Services. Prepared in accordance with FAR Part 12 Commercial Items and FAR Part 13 Simplified Acquisition Procedures as supplemented with additional information included in this notice. Joint Base Lewis-McChord, Health Contracting Branch (JBLM-HCB) is requesting product catalog and pricing as part of the Blanket Purchase Agreement (BPA) for FDA approved Blood and Blood Products for the Medical Treatment Facilities (MTF) and other related blood supplies for treatment of patient care; from interested, FDA approved Vendors, both Large and Small Businesses. See the attached Blanket Purchase Agreement Scope of Work and contact JBLM-HCB via email, if interested. ALL PRODUCTS MUST BE FDA APPROVED, AND MEET THE SURGEON/PATIENT REQIREMENTS UPON SELECTION. If vendors are other than the Original Manufacturers (OM), please provide evidence that there is an existing and ongoing business relationship that will allow distribution of the OM products. LOCALITY REGION RESTRICTIONS ** Interested Vendors must reside in the following locations; Pierce – Thurston and King Counties to provide prompt same day Blood Product deliveries in an urgency, emergency situation request to be in consideration. ** The Primary NAICS Code is 621991 Blood and Organ Banks and Secondary NAICS Codes 325414 is Biological Products (except diagnostic) Manufacturing; FSC Code is 6505. These products are to support the following Medical Treatment Facility, (MTF) and outlying clinics: Madigan Army Medical Center, (MAMC), Joint Base Lewis McChord, WA and all outlying Medical Clinics. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2020-4. Simplified Acquisition Procedures will be followed.
