Synopsis: The purpose of this sources sought notice is to determine if responsible small or large business sources exist capable of fulfilling the needs identified in this effort, for market research purposes only. The proposed North American Industry Classification System (NAICS) Code is 621511 with a corresponding NAICS Size Standard of $41,500,000.00. The Government will use this information to determine the best acquisition strategy for this procurement. The Government is interested in all small businesses to include 8(a) Program, Service-Disabled Veteran-Owned Small Business (SDVOB) Procurement Program, HUBZone Program, and Women-Owned Small Business (WOSB) Program concerns IAW FAR 19.203. The Medical Readiness Contracting Office - West, Fort Sam Houston, Texas, is seeking potential sources to provide the Laboratory and Radiology Services in support of the Occupational Health Clinic, McAlester Army Clinic USAOHC, MCAAP, Bldg. 5, 1 C-Tree Road, McAlester, OK 74501. McAlester Army Clinic supports a small population of military personnel, their families, veterans, and beneficiaries. Located 150 miles from Dallas, Texas, 150 miles from Oklahoma City, Oklahoma, and more than 90 miles from Tulsa, Oklahoma, timely laboratory and radiology services are needed to support the population. The Occupational Health Clinic requires laboratory and radiology services for the testing of CXR, CBC, Urinalysis; Liver Profile; LDH: GGT; Lipid Panel; TSH; Chem 8 Panel; G6PD; Free T4; Free T3; Free Testosterone, Total; FSH; LH; PRL; SHGB; CMP; TG Antibody; TPO Antibody; Estradiol Levels; HIV Screen; Uric Acid; Cholesterol Total; Lead, Blood; Zinc Protoporphyrin; Magnesium; Cadmium; PSA; ABO Type; RH type; Fluoride; Peripheral Smear Referral; Chem 20; RBC cholinesterase; varicella/rubella/rubeola titers, methemoglobin, HBsAb, occasional special labs to include urine heavy metals, Sars-COV-2 PCR Test, and reading of chest x-rays. (See attached PWS with detailed Scope of Work) This requirement is expected to result in the award of a single Firm-Fixed Price contract. This is a follow-on requirement to the previous contract no. W81K00-l 7-P-0736. The anticipated period of performance is a base year with four option years.
