The contractor will provide Ground Ambulance Services at Fort Polk, LA under the Performance Work Statement in the SF1449, supporting continuous 24/7/365 emergency medical services. Offerors must complete the SF1449 and required provisions, and be registered in SAM to be eligible for award. The technical proposal must demonstrate capabilities, staffing and personnel qualifications, vehicle and equipment maintenance, and quality management/performance assurance, including contingency, safety, training, and regulatory compliance plans. Past performance requires Project Summary Sheets for up to three recent and relevant contracts or task orders completed or to be completed within the past three years, with optional PPQs/CPARs and possible government validation through interviews or databases. Pricing must be completed for each contract line item on the Pricing Worksheet.
