The contractor will provide continuous 24/7/365 ground ambulance services at Fort Polk, LA under the Performance Work Statement. Offerors must submit a technical proposal addressing technical capabilities and contingency operations, personnel qualifications and staffing, vehicle and equipment maintenance, and quality management/performance assurance. Past performance must demonstrate recent relevant work by the offeror, using up to three project summary sheets for qualifying efforts within the past three years. Pricing must be completed for all contract line items on the pricing worksheet. Offerors must be registered in SAM to be eligible for award.

Offerors must complete specified SF1449 blocks/provisions and be registered in SAM to be eligible for award. The technical proposal must address technical capabilities, personnel qualifications/staffing, vehicle/equipment maintenance, quality management, and transition/start-up timelines from contract award through full operational capability, including continuous 24/7/365 emergency medical services. Past performance requires evidence of three recent/relevant contracts or task orders performed by the offeror within the past three years, with optional PPQs/CPARs. Price must be completed for each CLIN on the Pricing Worksheet.

The contractor shall provide ground ambulance services at Fort Polk, LA, including continuous 24/7/365 emergency medical services under the Performance Work Statement and applicable federal, state, DoD, DHA, Army EMS, OSHA, and HIPAA requirements. Offerors must be registered in SAM and submit a completed SF1449, required provisions, and pricing for all CLINs, with prices held firm for at least 90 days. The technical proposal must address technical capabilities, personnel qualifications and staffing, vehicle and equipment maintenance, and quality management/performance assurance, including contingency, continuity, and transition/start-up timelines. Past performance must demonstrate the offeror’s recent and relevant same/similar services within the past three years using up to three Project Summary Sheets, with optional PPQs or CPARs accepted. Proposals must be submitted by email to the specified Government contacts, and the Government may validate past performance through interviews and other sources.
