This is a Combined Synopsis/Solicitation for commercial supplies prepared in accordance with the format in FAR Subpart 12.6 and using the procedures of FAR Part 13--Simplified Acquisition Procedures, as supplemented with additional information included in this notice. The solicitation number is W81K00-23- Q-0072 and it is issued as a Request for Quote (RFQ). This announcement constitutes the only RFQ that will be posted. This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2023-02 and the Defense Federal Acquisition Regulation Supplement Publication Notice (DPN) 20230322. This solicitation is for ManoScan ESO Z Catheter 3890 or Equal for Evans Army Community Hospital, Fort Carson, Colorado. The North American Industrial Classification System (NAICS) code for this procurement is 339112 with a size standard of 1000 employees. This acquisition is conducted as a small business set-aside. Solicitation incorporates by reference FAR 52.212-1, 52.212-2, 52.212-3, 52.212-4 & 52.212-5 are attached. Addenda are attached. Offers are due on or before 01 May 2023/10:00 AM Mountain Time.
