COMBINED SYNOPSIS/SOLICITATION W81K00-22-Q-0191 This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6 and using the procedures of FAR Part 13--Simplified Acquisition Procedures, as supplemented with additional information included in this notice. The solicitation number is W81K00-22- Q-0191 and it is issued as a Request for Quote (RFQ). This announcement constitutes the only RFQ that will be posted. A separate written solicitation document will be not be issued. This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-07 and the Defense Federal Acquisition Regulation Supplement Publication Notice (DPN) 20220623. This solicitation is s for Preventive Maintenance (Full Service Plan) for GentleMax Pro Laser and VBeam Laser Perfecta System located at Reynolds Army Community Hospital, Fort Sill, Texas. The period of performance is 1October 2022- 30 September 2023 plus four 12-month options. The North American Industrial Classification System (NAICS) code for this procurement is 811219 with a size standard of $22M. This acquisition is a 100% Small Business Set-Aside. All eligible businesses may submit an offer, which will be considered. Solicitation incorporates by reference FAR 52.212-1, 52.212-2, 52.212-3, 52.212-4 & 52.212-5 are attached. Addenda are attached.
