The Soliciation Amendment is to Extend the closing date to 14 August 2020, 4:00 PM CST This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and FAR Part 13 Simplified Acquisition Procedures, as supplemented with additional information included in this notice. The solicitation number is W81K00-20-Q-0195 and it is issued as a Request for Quote (RFQ). This announcement constitutes the only RFQ that will be posted and a separate written solicitation document will not be issued. This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-07 and the Defense Federal Acquisition Regulation Supplement Publication Notice (DPN) 20200605. This RFQ is for the purchase and delivery of medical gases at two (2) separate locations. The primary location is Evans Army Community Hospital, 1650 Cochrane Circle, building 07500, and Premier Medical Home CMBH, located in the Premier Health Plaza on the corner of Austin Bluffs and the Union boulevard overpass, 3920 N. Union Boulevard, Suite 100, Colorado Springs, Colorado 80907. The Contractor shall provide all labor and materials to deliver and refill Government and Contractor-owned gas cylinders, provide the required equipment and testing. The Point of Contact (POC) for this RFQ is Ms. Kimberly Macias. Questions shall be submitted to the POC via e- mail at kimberly.r.macias.civ@mail.mil not later than 3 August 2020; Questions will not be accepted after this date (Refer to Addendum to FAR Provision 52.212-1). Quoters shall reference the following FAR Provisions and their Addendums to prepare and submit their quote: (a) FAR Provision 52.212-1-Instructions to Offerors—Commercial Items and its Addendum; and, (b) FAR Provision 52.212-2 Evaluation—Commercial Items and its Addendum. Quotes are due on or before 5 August 2020 at 12:00 PM Central Time.
