The purpose of Amendment 0004 to Solicitation W56HZV-21-Q-0109 is to: 1. Extend the offer due date to 3 February 2023 at 5pm EST. 2. Remove the unexercised option CLIN 0002AA. 3. Update clause 52.211-8: TIME OF DELIVERY to reflect removal of the option quantity. 4. Remove clause 52.217-7: OPTION FOR INCREASED QUANTITY--SEPARATELY PRICED LINE ITEM. 5. Update Narratives A, L, and M to reflect the removal of the option CLIN. 5. All other terms and conditions of Solicitation W56HZV-21-Q-0109, except for those addressed by Amendment 0004, remain in full force and effect. The purpose of Amendment 0003 to Solicitation W56HZV-21-Q-0109 is to: 1. Extend the solicitation due date to 21 July 2022 at 5pm EST. 2. Update the quantity to 18ea for CLIN 0001AA and 0002AA. 3. Update 52.211-8 Time of Delivery to reflect the updated quantity of CLIN 0001AA and 0002AA. 4. Add clause 52.217-7 OPTION FOR INCREASED QUANTITY--SEPARATELY PRICED LINE ITEM 5. Add clause 52.215-20 Requirements for Certified Cost or Pricing Data and Data Other Than Certified Cost or Pricing Data 6. The purpose of Amendment 0002 to Solicitation W56HZV-21-Q-0109 is to: 1. Decrease the quantity from 23 ea to 15 ea for CLIN 0001AA and 0002AA. 2. All other terms and conditions of Solicitation W56HZV-21-Q-0109, except for those addressed by Amendment 0002, remain in full force and effect. The purpose of Amendment 0001 to Solicitation W56HZV-21-Q-0109 is to: 1. Increase the quantity from 15ea to 23 ea for CLIN 0001AA and 0002AA. 2. Change the part number from 20001107-50 to 5-15-33943-50. 3. Change CLIN 0001AB, First Article Test & Report from Inspection and Acceptance at Destination to First Article Test & Report Inspection and Acceptance at Origin. 4. Extend the offer due date and time from Thursday, 20-Jan-2022 at 1 p.m. Eastern Standard Time (EST) to Friday, 18-Feb-2022 at 1 p.m. EST. 5. All other terms and conditions of Solicitation W56HZV-21-Q-0109, except for those addressed by Amendment 0001, remain in full force and effect. 1. Request for Quotation (RFQ) will result in a Firm Fixed Price purchase order for the following item: NOUN: Tubing Assembly Set PART NUMBER: 5-15-33943-50 NSN: 4710-01-549-6803 Unit of Issue: Each Option: 100% (UNEXERCISED) 2. First Article Test (FAT) and Production Lot Test (PLT) are required for this item. 3. There is a Technical Data Package (TDP) associated with this requirment. It is Distribution code "D", which limits access to Department of Defense (DoD) & U.S. DoD contractors only. Please refer to section C for additional information and directions on how to access this document. 4. The Government requires Military Packaging - Level B and has associated Special Packaging Instruction (SPIs) for this procurement. Please refer to Section D in the solicitation for additional information. 5. The Surveillance Criticality Designator (SCD) is "B" 6. The contractor is authorized to ship early at no additional cost the Government. All shipments will be FOB Destination. 7. This solicitation contains provisions and clauses that require you, the Offeror, to complete fill-ins and/or representations. Please review and complete all such fill-ins. Your offer may be determined ineligible for award if these fill-ins and representations are not completed. 8.
