This solicitation is prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. 1. The U.S. Army Contracting Command - Rock Island (ACC-RI) hereby Requests a Proposal (RFP) W519TC-23- R-2060 for the procurement of Prevention Program Coordinator Services for all personnel, supervision, access to prevention resources and non-personal services for the U.S. Army Garrison-Rock Island Arsenal (USAG-RIA), in accordance with (IAW) Attachment 0001 - PWS ASAP Services, dated 26 July 2023. 2. This solicitation is issued as a 100% Small Business Set-Aside. The RFP will be Firm Fixed Price (FFP) and IAW the provisions and clauses included in this solicitation. The resultant award will be a FFP Contract. 3. The U.S. Government intends to make a single award to the Offeror as outlined in FAR Clause 52.212-2 - Evaluation Commercial Items. The Government reserves the right to award no contract at all, depending on the quality of the quotes, prices submitted, and the availability of funds. 4. Offerors proposal submittal shall include a completed Attachment 0002 Pricing Matrix to include the base and all option periods. The Technical and Past Performance submittals as outlined in FAR 52.212-2 in the solicitation, page 19. Please confirm that you do not take any exceptions to the RFP. 5. Offeror shall submit Prime for Life Instructor certification or shall obtain a prime for life instructor certificate within 60 days of contract award in accordance with (IAW) PWS Section 2.3.4. 6. This will result in a five (5) year contract consisting of a one-year base with four (4), one (1)-year evaluated option periods. There is a six (6) month extension evaluated option if FAR 52.217-8, Option to Extend Services, is exercised. The dates below are subject to change based on award date and base and options are 365 days in length. Period of Performance (PoP) is: Base Period: 01 September 2023 – 31 August 2024 Option Period 1: 01 September 2024 – 31 August 2025 Option Period 2: 01 September 2025 – 31 August 2026 Option Period 3: 01 September 2026 – 31 August 2027 Option Period 4: 01 September 2027 – 31 August 2028 6 month extension: 01 September 2028 – 31 March 2029 7. The resultant Contract will be paid via Wide Area Work Flow IAW DFARS 252.232-7006. 8. The following NAICS code and Product Service Code (PSC) apply: NAICS: 621420 PSC: G004 9.
