Requirement Title: 125 STS Human Performance Program Services This project is located at the 142d Wing, Portland Air National Guard Base, 6801 NE Cornfoot Rd, Portland, Oregon. This project is being solicited 100% Small Business Set Aside. The North American Industry Classification System (NAICS) code for this work is 621399. The small business size standard is $8 million. Due to COVID-19 precautions and the nature of the services required there will not be a Pre-Proposal Conference or a Site Visit. This is not an Invitation for Bid (IFB), offers will not be opened publicly. Funds are not presently available for this contract. Currently, the Government anticipates funds will become available prior to but no later than midnight 30 Sep 2021. This action has been identified on the National Guard priority list for award if funds become available and the prices received are within an awardable range. Award will be made to the offeror who represents the best value to the Government as outlined in accordance with the criteria set forth within the solicitation. Document Type: Pre-Solicitation Notice Solicitation Number: W50S8Y-21-R-5001 Classification Code Q518 – Medical Physical Medicine/Rehabilitation Set Aside: Total Small Business NAICS Code: 621399 – Offices of All Other Miscellaneous Health Practitioners The Oregon Air National Guard at Portland Air National Guard Base intends to issue a Request for Proposal (RFP) to award a single firm fixed- price contract for services, non-personal, to provide all personnel, equipment, tools, materials, supervision, and quality control necessary to perform Human Performance Program (HPP) services. The Contractor shall perform injury prevention and mitigation, rehabilitation services, and human performance optimization services to personnel assigned to the 125 Special Tactics Squadron (STS), 142 Fighter Wing (FW), Portland, OR, USA. The Contractor shall furnish consultation and reports, similar to the range of services found in a commercial facility, as well as services specialized to meet the needs of Air Force Special Warfare operators. Performance shall be in accordance with the professional standards of the Accreditation Association for Ambulatory Health Care (AAAHC), and as defined in the Performance Work Statement. The North American Industry Classification System (NAICS) code for this work is 621399. The small business size standard is $8 million. This action is being procured on a total small business set-aside basis. The tentative date for issuing the solicitation is on-or about 6 April 2021. Due to COVID-19 precautions and the nature of the services required a site visit will not be conducted.
