The 137th SOW Will Rogers Air National Guard (WRANGB) is prepared to award a Blanket Purchase Agreement (BPA) to a vendor for the Disposal of Medical Waste at the 137th SOW. This BPA would be an agreement between your company and 137th SOW for 5 years after receipt of award (ARO). The BPA allows the OKNG to come directly to your company based on the Firm-Fixed Price provided in your proposal. Period of Performance is TBD 5 years ARO. Contractor shall conduct Medical Waste Disposal IAW with Performance Work Statement and all Federal, State, and Local laws, regulations, and policies. Pricing will include a per pickup cost with the provision of 2 disposal containers and replacement containers with liners; each container disposal weight will not exceed 40lbs. The pricing will include, but not limited to, a Firm Fixed Price for the turn key service to provide proper waste containers, maintain pickup and delivery services with transportation, and disposal of all medical waste at the 137th SOW. At a minimum, contractor shall respond by completing page 2 and providing pricing on CLIN 0001 found on page 3, of the attached Combined Synopsis/Solicitation W50S8T21R0008. The attached document also contains the PWS and Terms/Conditions for review. Contractor is required to sign this document and return 1 copy to issuing office. Contractor agrees to provide all services set forth or otherwise identified above and on any additional sheets subject to the terms and conditions specified herein.
