This requirement is for a Firm Fixed-Price (FFP) contract for the turn-key installation of one (1) each Computer Numerical Control (CNC) Solid State Laser Cutting System, to include: all necessary engineering and design; all necessary site preparation work, such as machine removal, excavation work, and concrete and isolation pad installation; and the supply, delivery, installation, documentation and personnel training required. See Attachment 1 - SOW for full description of requirement. This is a combined synopsis/solicitation for a commercial product prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the ONLY SOLICITATION; proposals are being requested and a written solicitation (SF1449) is provided with this notice. The following information is provided: 1. This solicitation is issued as a request for proposal (RFP) on the attached SF1449. 2. See SF1449 for all applicable FAR provisions and clauses. 3. This is a 100% Total Small Business Set-Aside under NAICS 333517, with a small business size standard of 500 employees. 4. See solicitation and attachments for CLIN information, description of requirement, delivery information, instructions to offerors (including FAR 52.212-1 and addendum), evaluation criteria and basis of award, and reps and certifications to be completed by offerors. 5. Site Visit and Visitor COVID-19 Mitigation Requirements: TYAD is currently operating under HPCON Level C, and as a result, COVID-19 Mitigation Requirements are in place for all visitors. All site visit attendees are required to: (1) Complete and bring with them the On-Site Visitor Health and Vaccination Questionnaire; (2) Provide a completed DD Form 3150 – Visitor Vaccination Certification OR a negative test result from a FDA authorized or approved COVID-19 test, administered no greater than 72 hours prior to their visit. The vaccination questionnaire and the DD Form 3150 will be provided upon site visit reservation confirmation (see solicitation, addendum to FAR 52.212-1, paragraph 5 for full site visit information). Failure to provide a completed DD Form 3150 or a negative COVID-19 test will result in being denied access to TYAD. Masks are mandatory. Amendment 0001 - Extend offer due date from 3 July 2022 to 5 July 2022 and incorporate revised site visit language. Amendment 0002 - Incorporate Attachment 3 - Site Visit Attendance Sheet. Amendment 0003 - Incorporate Attachment 4 - Questions and Answer V1 and Exhibit C - Laser Floor Area into the solicitation. Amendment 0004 - Extend offer due date from 5 July 2022 to 12 July 2022.
