To update and replace Attachment 1 - Price Schedule with the attached Attachment 1. Offerors must use and submit the Attachment 1 provided with this amendment when submitting their proposal. NOTE: CLINs NR01, NR02, NR90, NR93, NR94, and TX98 have been removed from the Price Schedule; CLIN NR05 has been added to the Price Schedule. 2. To update and replace Attachment 2 - PWS with the Attachment 2 - PWS attached to this amendment. NOTE: Sections 8.1 and 8.2 a. have been updated. 3. To provide the attached DLA Form 2505 which will be incorporated into the contract. This form has been updated into an Excel format to facilitate automations that are being developed. 4. To answer bidder questions. NOTE: There are three attachments (referenced above) within the one 'SP450023R0015 Amendment 0001' document that is attached to this notice. Offerors must complete Blocks 8 and 15 of the Standard Form 30 and include this form when submitting their proposal. ********** Combined Synopsis/Solicitation SP450023R0015 has been prepared in accordance with the procedures in FAR 12.603. The scope of work requires services to remove, transport, recycle, and dispose of industrial waste as identified in the U.S. Government (USG) and Japan Environmental Governing Standards (JEGS) and wastes regulated by Government of Japan (GOJ) as Industrial Waste (IW), Specially Controlled Industrial Waste (SCIW), Specially Handled Industrial Waste (SHIW), and Per- and Polyfluoroalkyl Substances (PFAS) located in and around Okinawa, Japan. The following additional information is provided in accordance with FAR 12.603(c)(2). (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation SP450023R0015 is issued as a request for proposal (RFP). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2023-04 Effective June 2, 2023. (iv) This acquisition is being issued as a full and open procurement; the associated NAICS code is 562211; the small business size standard is $41.5M. (v) See Attachment 1 for the Price Schedule. (vi) See Attachment 2 for the Performance Work Statement (PWS) and a description of the requirement. (vii) The contract is anticipated to have a 30-month base period from December 2023 to June 2026 and a 30-month option period. See Attachment 3 for a list of pickup locations.
