Each unit shall include a urinal and toilet. Period of performance is for a base of twelve (12) months from 01 May 2019 through 30 April 2020 with three (3) One-year option periods. PORTABLE TOILET UNIT LOCATIONS: AMMO AREA BLDG S622, SM ROAD 3: ONE (1) UNIT AMMO Area bldg. 614, SM Road 3: One (1) unit OTC Area North Open Lot: Two (2) units OTC Area South Open Lot: Two (2) units Bldg. 550: One (1) Unit Area 28 (Sea Container Area): One (1) unit RRAD West: One (1) unit Bldg 303E: One (1) unit Bldg 597A: One (1) unit Waste shall be removed and the unit shall be cleaned twice weekly. The contractor shall provide portable toilets, all personnel, equipment, tools, supervision and other items necessary to perform scheduled service and maintenance on eleven (11) each portable toilets in accordance with the Scope of Work. The requirement is described in Attachment 1 - Scope of Work. Any award resulting from this RFQ will be issued on a Standard Form (SF) 1449. The attached Wage Determination NO: 2015-5145 REV 7 dated 12/26/2018 is applicable to this purchase order. This effort will be awarded as a Firm Fixed Price purchase order. CONTRACTOR MANPOWER REPORTING: The Contractor shall report all contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Defense Logistics Agency via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address: http://www.ecmra.mil. Reporting inputs will be for the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be report no later than October 31 of each calendar year, beginning with 2013.
