This combined synopsis/solicitation (CSS) is being issued as a limited source action under NAICS 339115 with a size standard of 1000 employees and PSC 4240 Safety and Rescue Equipment SP3300-25-Q-0020 is being issued to establish a Firm Fixed-Price contract for Eye Examination and Prescription Safety Glasses for DLA Distribution Susquehanna PA (DDSP) and DMFS Installation Management. The optometrist is required to be located with a 20-mile radius of DLA Distribution Susquehanna and have an active SAM registration. The Government intends to award one (1) contract as a result of this CSS which will be issued on a Standard Form (SF) 1449. See Attachment 1- Statement of Work. Warehouse personnel and other mission essential employees require safety glasses to perform their essential functions properly in the work environment. Without proper eye wear, operational risks will increase significantly and degrade mission readiness. The period of performance is for a base plus 4 option years. Quantities are based off the number of employees at DDSP and DFMS who require safety glass prescriptions.
