Reference is made to Special Notice to contract SP3300-17-P-0795, published to SAM.gov on 6/2/2022. This notification is being published to incorporate a copy of the redacted Limited (Sole) Source Justification Memo, resulting from executed modification P00010 to the referenced contract. The anticipated three-month extension was reduced to a two-month extension (period of August 1, 2022 through September 30, 2022). The original Special Notice read as follows: "Occupational Health Services for DLA Distribution San Joaquin, CA (DDJC) General Information Document Type: Special Notice (Sole Source Memo) Solicitation Number: Ref. SP3300-17-Q-0058 (Contract SP3300-17-P-0795) Posted Date: May 5, 2017 NAICS Code: 621498 Set-Aside: 100% Small Business PSC: Q201 Description DLA Distribution, New Cumberland, PA intends to extend services in accordance with (IAW) FAR clause 52.217-8, Option to Extend Services, through a modification to the existing DLA Distribution Firm-Fixed Price (FFP) contract number SP3300-17-P-0795. This contract was established for a contractor to provide Occupational Health (OH) Support Services for DLA Distribution and tenant personnel located at DLA Distribution San Joaquin, California (DDJC). A follow-on contract is being recompeted at this time. In order to ensure continuity of services while the follow-on contract is awarded, a short-term extension of the current contract may be required. The extension will be in accordance with FAR 52.217-8, “Option to Extend Services” clause that is included in the contract. In accordance with FAR 6.302-1, DLA anticipates awarding a modification to existing DLA Distribution Contract SP3300-17-P-0795 on a sole source basis to Corky Hull Medical Associates, Inc. The period of performance extension will be three (3) months beginning on August 1, 2022 through October 31, 2022. Pursuant to the exception under FAR 6.302-1(a)(2)(iii), only Corky Hull Medical Associates, Inc. is capable of providing the services at this time; award to any other source will result in unacceptable delays and would preclude DLA from reacting timely to requirements in support of the mission at DDJC. This action could not have been overcome by advanced planning given the nature of this contract and the unexpected notification by DLA of exclusion of the required services on an Integrated Enterprise-wide contract, just a few months prior DLA Distribution’s contract expiration. If the contract is not extended to accommodate the known and anticipated requirements of this contract, DLA Distribution would not have a contract vehicle to receive necessary services within the scope of this contract to support DDJC and its mission. The modification to current contract SP3300-17-P-0795 will serve as a temporary solution to fulfill immediate mission requirements. The Limited (Sole) Source Justification for the three (3) month extension period will be posted as part of this notice once executed. This notice of intent is not a request for competitive proposals/quotes. This is not a solicitation for proposals/quotes or an invitation for bids. This synopsis is for informational purposes only.
