Dental Surgical Service
The Indiana National Guard seeks a contractor to provide surgical dental services to soldiers, including extractions, crowns, root canals, and associated procedures, at the contractor’s facility within 50 miles of Camp Atterbury, Indiana. The contract is an unpriced purchase order with a not-to-exceed amount of $100,000, with billing handled via WAWF after treatment is completed. Contractors must accept Concordia and Blue Shield Blue Cross insurance, bill those insurers first, and invoice the government for any remaining balance or for uninsured soldiers. Deliverables include extensive dental treatment, a monthly tracker of treatment and costs, and completed DD 2813 forms showing soldiers’ dental readiness codes. Proposals will be evaluated on location, past government contracting experience, acceptance of the specified insurances, and ability to complete all Performance Work Statement tasks; a retail price sheet is requested, but pricing will carry minimal evaluation weight.
