CCAM-CAD-C 08 July 2021 SUBJECT: Request for Quote (RFQ) for onsite service for repair, calibration and software reconfigurations for one (1) an OPG Smartscope Quest 600 in support of U.S. Army Test Measurement and Diagnostic Equipment Activity (USATA) PERIOD OF PERFORMANCE: 90 days after receipt of order On behalf of the Contracting Officer, please accept this letter as official Request for Quote (RFQ) in support of the requirement described herein. Your company's electronic quote shall be submitted on company letterhead inclusive of the following: 1. Technical, delivery and performance specifications in accordance with (IAW) Attachment 001, Performance Work Statement. Quote shall include cost breakdown as well as clearly identify the grand total cost. 2. Business size classification and five (5) digit Commercial and Government Entity (CAGE) Code that allows your organization to conduct business with the Federal Government. Please note that CAGE Code must be active at contract award through performance completion. For more information on CAGE Codes please visit https://www.sam.gov. 3. If the opportunity to submit a quote is declined, please provide a "no bid" email response briefly stating the reason for the decline. Responses shall be sent via email to the Contract Specialist identified below. 4. Quotes must be submitted no later than 2:00 pm Central Standard Time (CST) Wednesday, 21 July 2021 via email to Contract Specialist, Jewel Loper, email: jewel.c.loper.civ@mail.mil. 5.
