Ser... This notice serves as a modification to Notice ID# PANRSA-20-P-013561. This contract provides all system hardware and software service and repair for the three LifePak 15s and Two LUCAS 2 Devices. Services shall include annual preventative maintenance inspections, documentation for regulating bodies, loaner devices during PM or repair, software updates, 24/7 telephone support, and OEM parts. The LifePak 15 Cardiac Monitors are a combined cardiac monitor/defibrillator which feature a dual-mode LCD screen with SunVue display; data connectivity; AED mode; and manual mode. They weigh 18.5 lbs and are powered by a Lithium Ion battery. The monitor is capable of 3, 7, and 12 lead ECG examination. Additionally, the monitor can measure SpO2, SpCO, SpMET, C02, pulse rate, blood pressure, and temperature. It contains a biphasic waveform defibrillator and is capable of cardiac pacing. The LUCAS 2 is an automated chest compression device that consists of a back plate; upper portion; stabilization strap; patient straps; and a carrying case. Please read the attached Statement of Work (SOW) for a complete description of the requirement, period of performance, contractor qualifications, etc. MODIFICATION DETAILS: This notice requires all quotes to include a base year plus four (4) option years. Please ensure that the cost breakdown for the base and each option year, in addition to the overall total, is included in the quote.
