Notice of Intent to Establish a Blanket Purchase Agreement BPA Estimated Dollar Limit: $100,000.00 BPA Call Limit: $10,000.00 The U.S. Army Corps of Engineers, Portland District intends to establish blank purchase agreement(s) (BPA) for occupational health services for Bonneville Lock and Dam (OR) employees. This notice to establish a list of potential vendors who are active in the System for Award Management and interested in establishing a BPA. This will be the only notice. Interested Clinics must be located within 25 miles of Cascade Locks, Oregon. To be eligible for consideration, the clinic must have on staff Occupational Health Physicians (OHPs) that are licensed and bonded as Board Eligible (BE) and/or Board Certified (BC) Doctors of Medicine. Interested Clinics must provide pricing for the services outlined in the price schedule and provide documentation showing they meet the criteria of the Performance Work Statement of Section 10.2.
