Notice of Combined Synopsis/Solicitation The Naval Supply Systems Command Fleet Logistics Center (NAVSUP FLC) has a requirement purchase for the U.S. NAVAL SUPPORT ACTIVITY in Souda Bay, Greece. This is a request for quote (RFQ) utilizing the procedures of FAR Part -12 Acquisition of Commercial Items & Supplies and Subpart - 13.5 Simplified Procedures for Certain Commercial Items. The resulting award will be a Single Award, Firm-Fixed Price (FFP) contract. This Contract will provide Laboratory Testing Services in support of U.S. Naval Branch Health Clinic (NBHC), Naval Support Activity (NSA) Souda Bay. NBHC requires clinical laboratory testing and diagnostic services contractor’s support to provide services to include and not limited to skills and labor, required equipment for sample specimen handling, courier, consultative and analytical testing, laboratory testing, writing test results reporting and delivery of documents. Per 52.212-1 ADDENDUM Each Quoter must submit a Quote (in English) as stated below: 1) Cover Sheet to include the following: -Company Name -Point of contact information -CAGE/NCAGE Code -Total quote amount in Euro - Statement the Quoter agrees to hold the prices in its quote firm for 30 calendar days from the date specified for receipt of Quotes. 2) Pricing-Exhibit A filled in its entirety. Proposed prices shall be in EURO. 3) FAR provisions and clauses Exhibit B filled in as required. 4) Copy of License of Operation 5) Copy of Certificate of Good Operation The Contractor shall be registered in the System for Award Management (SAM) database prior to the award of the contract. The Contractor shall provide proof of SAM registration; see FAR Clause 52.204-7 SYSTEM FOR AWARD MANAGEMENT.
