This is a combined synopsis/solicitation for commercial items prepared per the format in FAR Subpart 12.6, using Simplified Acquisition Procedures found at FAR Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations/offers are being requested and a written solicitation will not be issued. Solicitation N6809620Q1012-- is being issued as a Request for Quotation (RFQ). Solicitation N6809620Q1012-- and the incorporated provisions and clauses are those in effect through the Federal Acquisition Circulation 2020-08. This requirement will be procured full and open to the maximum extent practicable . The North American Industry Classification System (NAICS) code is 517312 –Wireless Telecommunications Carriers (except Satellite). Offerors must be registered in the System for Award Management (SAM) at http://www.sam.gov, in order to be considered for award. The line item number, description, quantity, unit of measure, and period of performance are detailed on page 3 and 4 of this notice / attachment. The work requirements are detailed in the performance work statement attached to this notice. Period of Performance: 30 September 2020 – 29 September 2021 (12 month Base Period plus one additional 12 month Option Period) Place of Performance: US Naval Hospital Guam, Building 50 Farenholt Road, Agana Heights, Guam 96910 The Supplies / Service are for the US Naval Hospital Guam (hereinafter referred to as USNH Guam). The Information Management Department USNH Guam will inspect/accept the supplies delivered/services performed. The provision at FAR 52.204-24, Representation on Contracting Telecommunication and Video Surveillance Services or Equipment applies to this solicitation and is incorporated in full text on page 4. The contractor must complete the representation at subparagraph (c) of the provision as part of it offer/quotation. The provision at FAR 52.212-2, Evaluation--Commercial, applies to this acquisition and is tailored as follows: Addendum to FAR 52.212-2: (a) The government contemplates awarding a fixed price purchase order from this solicitation to the responsible offeror whose quotation/offer conforming to the solicitation is the lowest priced quotation/offer.
