This is a SOURCES SOUGHT NOTICE (not a pre-solicitation notice or solicitation pursuant to FAR Part 5) and is a request for information to determine the capability of potential sources and the availability of commercial sources pursuant to FAR Part 10, Market Research. This notice is issued by the US Naval Hospital Guam Contracting Department, to identify sources capable of providing document destruction (shredding) of paper products, including those containing Personally Identifiable Information. A Statement of Objectives is attached to this Sources Sought Notice. The documents for destruction will be collected by the service provider from established locations throughout the US Naval Hospital Guam facility. The anticipated contract vehicle for the document destruction services will be a Firm Fixed Price purchase order. The purchase order will be have a base 12- month period (starting from September 1st, 2019), and includes 4-twelve month option periods, that are unilaterally exercisable by the government. The purchase order will include all the terms and conditions governing the relationship between the document destruction service provider and the US Naval Hospital Guam. The purchase order will also contain the mutually agreed upon pricing for the document destruction services requested and to be provided. The purchase order, which will be signed by an authorized representative of the document destruction services provider and the Contracting Officer for the US Naval Hospital Guam. Any document destruction service provider interested and capable of providing these services is requested to respond via e-mail to eddie.l.franklin.civ@mail.mil by August 16th, 2019 at 8:00AM, Chamorro Standard Time (GMT+10) with the following information/documentation: 1. Company Name, Address and DUNS Number; 2. Point(s) of Contact and Telephone Number(s); 3. A Capability Statement that demonstrates the ability to meet the requirements in the Statement of Objectives. There are no funds associated with this Sources Sought Notice. This Notice is for market research purposes only and does not constitute a Request for Quotation or Request for Proposal; nor is it to be considered a commitment by the US Naval Hospital Guam. The US Naval Hospital Guam will not pay for any information provided by interested parties; nor is there a basis for a claim against the US Naval Hospital resulting from any response to this Sources Sought Notice or from the use of this information by the US Naval Hospital Guam. Statement of Objectives SUMMARY OF EXPECTATIONS: 1. The service provider shall be both Health Insurance Portability and Accountability Act (HIPPA) and National Association for Information Destruction certified. 2. The service provider shall provide all resources including personnel, equipment, lockable containers/bins, collection (from established locations within the US Naval Hospital Guam facility), transportation, and the destruction facilities. 3. All staff performing the services must be fully trained, professional, and customer service oriented, consistently provide responsive, flexible, and cost effective services. The service provider shall ensure quality work performance per thin accordance with applicable standards and guidelines shall be ensured. 4. The service provider is expected to partner with the US Naval Hospital in continuously improving the quality of services performed and the relationship between the client and the service provider.
