To be considered for this requirement, you must return a complete SF1449 and must be able to meet the coverage requirements listed in each Item as well as within the Performance Work Statement. No other format except the SF1449 will be accepted. For the SF1449 to be complete, please fill out the following on the SF1449 - On page 1, block 17a and blocks 30a-c. **Cursive font signatures won't be accepted.** Complete all of page 3. Insert pricing per Item starting on page 4.
